EXAMPLE ONLY  CrazyTrain L.L.C. Rental Agreement 2757 East Baseline Road Mount Pleasant, MI 48858 (989) 737-6581     CrazyTrain L.L.C. agrees to rent the CrazyTrain Bus to                                , Lessee, upon the following terms and conditions:   1.     CrazyTrain Bus rental from                    am/pm to                   am/pm on                        (mm/dd/yyyy). 2.     Lessee agrees to pay $                        dollars by money order, cash, or check at the start of the above trip plus a minimum of 15% gratuity to the CrazyTrain driver. 3.     Lessee will send a $100 dollar non-refundable deposit to the above address.  Once the deposit and this signed form are received the requested date will be reserved for the Lessee. 4.     There will be a $20.00 dollar fee on any returned checks.  5.     Lessee agrees to pay for any damages incurred to the CrazyTrain Bus as a result of any destructive behavior during the above-mentioned rental.  6.     Lessee understands that all State and Federal laws apply while on the CrazyTrain in regard to under-age drinking and illegal drugs.  Therefore, it is understood that neither are permitted. 7.     The CrazyTrain L.L.C. and its employees are not responsible for any lost or damaged personal items brought on the CrazyTrain Bus by the Lessee and/or rental party.     EXAMPLE ONLY                                                                                                     name, Lessee                                             Date
